- We use a SCOPE OF APPOINTMENT FORM, similar to a “Permission Slip,” which lists your name, the agent’s name, and the plan options you wish to discuss. These forms are typically required 48 hours in advance, except for “Walk-In” visits. (We can email or mail the form for your review and approval!)
