Well, maybe I should say Anthem Blue Cross of NH "reintroduces" the $5 Office Visit Copay. (If you are my age you remember when you could see a doctor for Five Bucks!) Welcome back $5 Copay!
During the past 5 years out-of-pocket costs for health care have continued to rise- making it less affordable for people to get treatment. (See KFF.org article here)
Today in the US, the average copay is $25 for Primary Care and $40 for specialist visits. However, in NH, it is common for employees to have office visit copays of $40 for Primary Care Doctor and $80 at the Specialist.
However, for the 3rd quarter 2019 Anthem Blue Cross Blue Shield in NH has introduced a new lower $5 Copay at the Primary Care provider with some of NH small and large employer group health plans.

Anthem calls this a "value based" model grades doctors on patient care, quality, reducing unnecessary or ineffective care and management of chronic diseases. Anthem's PREFERRED PRIMARY CARE DOCTORS have been identified as TOP PERFORMERS using this model
