$10 reward*If you're already going to a low-cost provider.$25 reward*If you save at least $150 by switching to a low-cost provider for:

• Mammogram
• Bone density study
• Ultrasound
• Lab Tests

$50 reward     **   If you save at least $250 by switching to a low-cost provider for:

• MRI
• CT
• PET scans

$75 reward     ***   If you save at least $350 by switching to a low-cost provider for:

• Colonoscopy
