When someone reaches Medicare age, there are two different ways they can obtain coverage:

1. Original Medicare PLUS a Medicare Supplement and Part D RX: This option involves enrolling in Original Medicare (Part A and Part B) and then adding a Medicare Supplement (Medigap) plan to cover additional costs. Additionally, a separate Part D plan is needed for prescription drug coverage. In New Hampshire, there are currently 20 different insurers offering Medigap plans and 20 different Part D RX plans to consider.
2. Medicare Advantage PD: This is an alternative way of receiving Medicare benefits through a private company. These plans, also known as Part C, often include Part A, Part B, and Part D coverage in one package. In 2024 in most counties in New Hampshire, there are over 35 different Medicare Advantage plans available.

Medicare Advantage Enrollment Trends in NH

Big news for anyone looking to buy Anthem Medicare Supplement Plans in New Hampshire. Starting April 18, 2020 New Hampshire's largest writer of Medicare Supplement plans, Anthem Blue Cross Blue Shield of NH, will begin to ask medical questions AND also offer tobacco/non-tobacco rates but ONLY IN CERTAIN CIRCUMSTANCES.



Good News Up Front
Not everyone applying will be subject to the new underwriting. Here are a few groups of applicants who will NOT NEED to answer questions, or be subject to tobacco rates. 

• NEW TO MEDICARE or in their INITIAL ENROLLMENT PERIOD 
• COMING OFF AN EMPLOYER GROUP PLAN 
• NEW to MEDICARE PART B
• Departing a Medicare Advantage Plan during the 1st year on that type of plan

The applicants in one of the scenarios listed above will be GUARANTEE ISSUE, meaning that they will NOT BE ASKED MEDICAL QUESTIONS and they will also not be subject to TOBACCO RATING.

So for people that are NOT within one of the GUARANTEE ISSUE scenarios, Anthem will Medically Underwrite their NH Medigap policy. In laymen's terms, Medical Underwriting means that  an applicant's approval- or - denial will be based upon their medical history and background. Anthem will ALSO be able to charge a higher rate for tobacco users.

What are the questions  that Anthem will ask..?
A review of the new application shows questions like...

• Are you currently confined, or has confinement been recommended to a bed, hospital, nursing facility...?
• Within the past two years, have you been:

                a. Hospitalized two or more times, been confined to a nursing home for a total of two
                weeks or longer, or been to the emergency room more than three times?
                b. Advised to have surgery that has not yet been done, or advised that you will need
                to be admitted to a hospital, skilled nursing facility or rehabilitation facility?

Silver Lining to New Rules
One nice feature to this new underwriting method, is that it will open up Anthem Blue Cross Blue Shield's  NH Medicare Supplement Plan G to people who can qualify medically. (In the past only NEWLY eligible medicare applicants were eligible for this popular plan).

The NH Insurance Department has released an EMERGENCY DECLARATION that will give people who are insured under a  EMPLOYER GROUP or MEDICAID INSURANCE PLAN to access care remotely for the following kinds of visits...

Eligible Providers:
 Physicians/Physician Assistants/APRNs/Clinical Nurse Specialists/Nurse Midwifes
 Certified Registered Nurse Anesthetists
 Clinical Psychologists, Clinical Social Workers, Master’s Level Psychiatric Nurses
 School Psychologists licensed by the Board of Psychologists
 Pastoral psychotherapists, Marriage and Family Therapists, Clinical Mental Health Counselors
 LADCs, MLADCs, CRSWs, Applied Behavioral Analysts
 Providers licensed by the Board of Mental Health Practice
 Dentists, Community Mental Health Providers
 Registered Dietitians or Nutritional Professionals

Originating Sites: There are no restrictions. These may include, but are not limited to: Patient’s Home, Practitioner’s Office, Schools, Hospitals, Renal Dialysis Centers, Skilled Nursing Facilities, FQHCs/RHCs, Community Mental Health Centers, or The Doorways.

Reimbursement: Commercial carriers are directed to pay the same rate as if the service was provided face-to-face. Billing for the service delivered should follow routine practices as if the service was provided face-to-face. Documentation standards follow the regular standards as if the service was delivered face-to-face.

As many of you know, the Annual Open Enrollment for Medicare begins 10/15/2019 and ends 12/7/2019. See 5 Tips for a successful 2020 NH Medicare Open Enrollment.

1. Keep an Eye out for your plan’s Annual Notice of Changes!If you are enrolled on a Medicare Part D RX PLAN or a Medicare Advantage Plan you will receive a notice form your EXISTING plan that outlines any cost or coverage changes for 2020. Plans are required to send this on or before September 30th.
2. Even if you are covered by an employer plan you MIGHT want to consider your Medicare options…The NH Market for Medicare plans offers more options each year- and with the increases in employer plan out-of-pocket costs some seniors find Medicare to be more attractive
3. Starting October 1st you can compare the 2020 version of current Part D RX plan with OTHER options If you have a Medicare Supplement with a SEPARATE PART D RX or a Medicare Advantage WITH PART  D RX please know that medications, costs, deductibles and coverage can change each year. Starting  October 1, 2019 you can visit the NEW MEDICARE PLAN FINDER  https://www.medicare.gov/find-a-plan  enter your zip code and RX -- this way you can check and see how your CURRENT PLAN and other plans will cover your medications in 202
4. Want to change the TYPE of Plan you have? This is what open enrollment is for.  Here is a quick list of changes that can be made at open enrollment. ​​​​                                                                                                           Anyone with Medicare Parts A & B can switch to a Medicare Advantage (Part C) plan.
            Anyone with Medicare Advantage (Part C) can switch back to Parts A & B.
          Anyone who has or is signing up for Medicare Parts A or B can join, drop or switch a Part D prescription     drug plan.
          Anyone with Medicare Advantage (Part C) can switch to a new Medicare Advantage plan.

Who likes to save money on healthcare? I think everyone does.

Formerly known as SaveOn, the Reduce My Costs program is available for MANY of the NH Group Health Insurance Plans  offered by Harvard Pilgrim Healthcare.

Simply stated, the Reduce My Costs program is a toll-free number  (1-855-772-8366) that an eligible Harvard Pilgrim Healthcare member can call to get help shopping for medical care, including:

• Lab testing
• Mammograms & Bone Density
• MRI's  & CT Scans
• Out-patient surgery
• Other non emergency services

With this program NH Harvard Pilgrim members can speak with an experienced nurse, discuss the cost of an upcoming service or test and compare the lower cost options that are available. 

If the member is already seeing a low cost provider they will get a reward just for calling. If the member decides to change providers and get care from a lower-cost provider, he or she will earn a cash reward ($$!) depending on the service and the associated cost savings.

In addition to saving money, one other nice feature Reduce My Costs program is that the Nurse will ALSO assist with scheduling or re-scheduling and with any paperwork required. 

This program used to be called SAVEON. See popular SaveOn article that I wrote a few years ago. This article described the ACTUAL EXPERIENCE of one of my clients who paid $270 vs $2000 for an MRI. This story was featured by Harvard Pilgrim in some of their ads and brochures.

​Well, maybe I should say Anthem Blue Cross of NH "reintroduces" the $5 Office Visit Copay. (If you are my age you remember when you could see a doctor for Five Bucks!)  Welcome back $5 Copay! 

During the past 5 years out-of-pocket costs for health care have continued to rise- making it less affordable for people to get treatment. (See KFF.org article here)  

Today in the US, the average copay is $25 for Primary Care and $40 for specialist visits. However, in NH, it is common for employees to have office visit copays of $40 for Primary Care Doctor and $80 at the Specialist.

However, for the 3rd quarter 2019 Anthem Blue Cross Blue Shield in NH has introduced a new lower $5 Copay at the Primary Care provider with some of NH small and large employer  group health plans.

As much as we'd like to have the power to predict the future, we can't. Our best course of action, however, is to rely on data that indicates what's most likely to occur down the road. Nobody likes the feeling of missing the boat.

With the right information from Hagerty—insurance quote data, the frequency in which cars are added to policies, auction activity, and more—we can start to paint a picture of when prices for a particular model are on the verge of ticking up. After crunching the numbers, here are the seven cars that are overdue for a price bump.

1985-89 Ferrari 328 GTB/GTS
Median value for cars in #3 (Good) condition: $57,350
These cars essentially doubled in value since the early 2000s. Still, any “affordable” Ferraris can be a dangerous trap—the repair bills for a beat-up example could equal half what you paid. That reality makes any “cheap” Ferrari a difficult investment, even if the car appreciates 25 percent in a reasonable time span.

Twelve-month average auction sale prices for the 328 GTB/GTS peaked in August 2016 at $116,078. Today, that figure is down 31 percent to $80,592—equal to four years ago. It’s possible that buyers who were previously priced out 3–4 years ago can now get back in the game. Quote activity was up 11 percent in the final three months of 2018, which is a good sign for current demand, and quote value was up 3.8 percent in that period.

Median value for cars in #3 (Good) condition: $16,700
Young buyers don’t seem to love the Spider S4, with 76 percent of quotes coming from Boomers and Pre-Boomers over the last 12-months, but that doesn’t mean it is doomed.

Spiders are being added to insurance policies at an increasing rate—between January ’18 and Jan ’19 the number added to policies increased 38 percent. Meanwhile, the average sale price at auction over this period increased 19 percent. The average quoted value of the car has been very consistent over the last 10 years, ranging only from $12,800–$15,800. It’s fair to say that these roadsters are due for value appreciation, after many years of chugging along in the background.

Median value for cars in #3 (Good) condition: $16,200
Surprisingly, 85 percent more Mercury Cyclones were added to policies in this past year than in the one prior, and the quoted values were up 33 percent over the same period. Additionally, the average auction sale price over the last 12 months is up 37 percent compared to the previous year. The median #2-condition value has remained unchanged for the last four years, which is a good indicator of stability.

1949-67 VW Beetle

Median value for cars in #3 (Good) condition: $14,900
 
Amongst cars with 100 or more annual quotes from Gen X and Millennials, the 1949–67 Beetle is the only vehicle generation from the ’40s or ’50s with more than 60 percent of quotes coming from these younger demographics.
When Hagerty updated prices last September, median prices for #2 (Excellent) cars increased 26 percent. As those price creeps past the point where buyers are willing to throw money around, and big sales start happening more frequently, expect Excellent-condition cars to become more desirable than ever before.

1968-72 Oldsmobile Cutlass

Median value for cars in #3 (Good) condition: $12,850
The Cutlass is tracking in the middle of the road for quote activity and the frequency in which it’s being added to policies, but the auction and private sale activity are strong. Why? Mainly because Barrett-Jackson offered a ton of them in Scottsdale this year (the 12-month count of cars coming to auction increased 33 percent versus when factoring in January results). Over the last year, average sale price is up 22 percent.

Median value for cars in #3 (Good) condition: $11,100
Around early 2017, auction prices for Magnums bottomed out, and the 12-month average sale price was $9682. Given the low price, these Dodges are flipping the script, possibly because of the rarity and appeal of a full-size family-hauler wagon with rear-wheel drive, available all-wheel drive, and up to 425 horsepower.

Insurance quote activity was flat until auction prices bottomed. Since January 2017 the 12-month sum of quotes increased 84 percent, while the average quoted price dropped 23 percent. Those are signs that more people are interested in the car because of its bargain value, but the buyers that are buying for collectibility and true value have yet to join the fray (and drive up prices).

Hagerty is starting to see people pay more at auction, though. Between February ’18 and Oct ’18 the average sale price at auction increased 50 percent, and the number of Magnums coming to auction is holding steady near the highest it has been (17 per year). As more and more people realize that Magnums are relatively affordable, look cool, and are very practical, expect demand to push prices further.

At a time when buyers are gravitating towards affordable, reliable, and fun cars from the ’80s and ’90s, the VW Golf is in a good position to appreciate. The positive data is rampant with quotes dominated by Millennials, who make up 56 percent of the requests—up 22 percent in just three months. That’s translating into more Golfs being added to policies, and that number is twice what it was this time last year. These increases have not yet translated into increased quoted or purchase values, which shows that buyers are still seeing them as bargain cars and trying to get in early. It shouldn’t be long before reality sets in and prices rise.

It is essential that RV owners complete a spring checkup to ensure a hassle free summer camping trip. Our RV insurance market partner, National General has provided us with this list of spring recommended checklist for our RV owning clients:

1. Exterior: The most frequently encountered issue with RV's in storage is water leaks. Inspect all the caulking to be sure it is not cracked or missing. This includes caulking around windows, doors, all roof vents and seams. Even the smallest crack will allow water to intrude, Wash the unit using proper cleaners so the surface is not damaged. Pay particular attention to the awning.
2. Interior: Give the interior a quick dusting and vacuum, checking all around inside for areas that may need cleaned.
3. Tires: Inspect tires for cracks on the sidewall and between treads. Replace tires if cracks are noted to avoid any blowouts while on the road. Check tire pressure per manufacturer recommended tire pressure which is located on a decal inside of your cabinet.
4. Appliances: Remove the outside access covers for the water heater and refrigerator. Inspect the burner assembly for any foreign debris such as spider webs and nests. Light air pressure from a compressor can be used to clean the burner area (do not blow high pressure up the refrigerator chimney).
5. Batteries: When in doubt please have a professional address. You should be wearing safety glasses and latex gloves, disconnect the shore power, have main cut off in off position before getting started:
   1. Clean corroded terminals, connections, and battery trays using hot water and baking soda.
   2. Check battery fluid levels where practical. Be careful with wiring if removing the batteries as positive and negative wires are not color coded often (use masking tape and a sharpie to flag). Remember that improper battery connections will lead to mass component failure. The goal here is to check batteries for fluid. Top fluid levels off with distilled water but pay attention as you don't want to overfill. Keep the level below the upper case fill ring.
   3. Have a professional complete a load test. Batteries must be fully charged before the test otherwise they will fail the load test. If the professional testing the battery does not check the state of charge, go elsewhere as the vendor is likely pushing to sell new batteries.
   4. If battery replacement is required always replace multiple battery banks together. Do not add an old battery to a new battery.
6. LP: Make sure the L.P. components turned off before proceeding forward. Turn the leak detector inside the RV on first and open the valve on the tank all the way. Smell around the area for leaks (can also use an electronic leak detector), using soapy water to check for leaks around the valve and regulator. If a leak is detected turn off the L.P. valve immediately and consult with a professional to repair the leak. Please note that sometimes seals dry out over the winter causing new leaks. Last but certainly not least, absolutely no smoking/flames/sparks while completing this step.
7. Water: The hot water heater bypass valve needs to be in the "normal" position and all faucets need to be closed prior to filling with water. Open a hot and cold water faucet during filling to allow the air to escape until the water flows steady. Inspect faucets, water heater over pressure safety valve, and the water heater drain plug for leaks.
8. Test fire appliances: With the L.P. gas leak detector on, start by lighting a stove top burner. This is the easiest flame to see, so you will know when it ignites and the air in the lines is purged. Light the other L.P. fired items, making sure the water heater is full before igniting.
9. Generator: Some newer generators have a prime function to run the fuel pump prior to starting. Hold the start-stop switch in the stop position, one the light in the switch is illuminated run the pump for about 20 seconds to deliver fuel to the carburetor. If the generator does not have a prime feature, you will have to crank it until it starts. Let the starter rest to cool after about 15 seconds of cranking. Cycle the starter until it runs. Let it run for a short time and check the oil level. Check the air filter to rule out any critters nesting from storage. Check the owner's manual for any other procedures.
10. Dump Hoses: Inspect the sewage dump hose. Dump hoses have a limited shelf life. Verify that the hose is in good condition with no tears or holes.
11. Waste tank valves: Run the waste tank valves while connected to a dump station. Valve seals can dry out making the valves difficult to operate. Slowly working the handle in and out in short increments may save replacing the valves. There are also additives for valve lubrication that can be added to the tanks.
12. Awnings: Simply operate awnings and inspect fabric. The spring tension may need to be adjusted if you have difficulty extending the awning. Spring adjustment is dangerous and we would highly recommend hiring a professional to address issues.
13. Water Heater: Remove the drain plug at the bottom of the tank and allow any sediment to escape with the water. Some models also have an anode rod that is part of the drain plug. With the tank drained check the anode for deterioration and rule out replacement. Inspect the rod and replace it if it is more than 75 percent depleted. Refer to the water heater's instruction manual for replacement details. Reinstall the drain plug and refill the tank before lighting the burner.
14. Freshwater Tanks: Should be sanitized by filling the water tank half full. Add a solution of 1/4-cup household bleach and 1 gallon of water for every 15 gallons of tank capacity. Open all the faucets until all air has been bled from the system and the water coming out has the odor of the bleach solution before shutting off the taps. After three to five hours, drain the water system and refill it with fresh water. Run water through all faucets and then drain the system again.
15. Radiator: Hoses to be replaced about every five years. Although they may look fine from the outside, many times they deteriorate inside first. They can then burst without warning and you fail to notice the temperature gauge, severe engine damage may result.
